Admissions Form
For all year groups, except for Reception, please complete the Admission Form and return it the school office.
If you are applying for Nursery please indicate if this is 2 year old or 3/4 year old Nursery.
Once in our 2 year old nursery, you will not need to reapply for 3/4 nursery but you will need to apply for Reception places through the Oxfordshire County Council when the time comes.
Reception Class
We are very pleased to announce that the academy’s enrolment figure is constantly on the way up! We are full in many year groups now. For those children who attend our Nursery, they must apply for a place in Reception through Oxfordshire County Council by filling in their CAPF form. These applications are dealt with the November – January before children are due to start the Reception class. We cannot guarantee a place in the Reception class so we urge you to get applications in on time and always state the three schools you would like your child to attend in order of preference.
Key dates when applying for September 2022 entry
If you do not receive a place in the Reception class, you have the right to appeal the decision by filling out the forms sent to you from the Local Authority. Once the OCC form is submitted, you will be asked to complete our Admissions Appeal Form (see below).

In-Year Admissions Form
CAPF Primary In-Year Admissions Form
For pupils applying to Dashwood Banbury Academy during the school year eg transferring from another school or moving to the area. Children in the academy catchment area going into Reception should go through the Local Authority Admissions process and should not use this form.
